Terms & Conditions

Effective date: September 25, 2026

These terms describe use of the MDConnect.info intake portal and the MDConnect SMS messaging program. For assistance, email contact@accidentservice.net.

1. Portal and appointment requests

Use the portal to submit accurate information and request an appointment or related assistance. Submitting the form records a request; it does not confirm an appointment. Our team will review the request and contact you. The website does not provide medical or legal advice and is not intended for emergencies. For an emergency, contact local emergency services.

2. SMS program and opt-in

If you opt in, MDConnect may send non-marketing text messages about intake status, appointments, appointment updates, and document requirements related to your request. Message frequency varies according to your needs and interactions with our team.

SMS consent is optional and is not a condition of submitting an intake form or requesting services. Selecting the SMS consent checkbox authorizes messages to the mobile number you provide. Only provide a number you are authorized to use. SMS consent does not authorize unrelated promotional messages.

3. How to stop messages

You can cancel the SMS service at any time by replying STOP to a message you receive from us. We may send one confirmation that you have been unsubscribed. After that, you will no longer receive messages from that program unless you opt in again. To rejoin, complete the opt-in process again. Unsubscribing from SMS does not cancel your appointment or intake request.

4. Help and support

If you experience an issue with the messaging program, reply HELP for assistance or email contact@accidentservice.net.

5. Message frequency and charges

Message and data rates may apply to messages sent to you and messages you send to us. Message frequency varies. Contact your wireless provider with questions about your messaging or data plan.

6. Message delivery

Delivery depends on your wireless provider and network availability. Carriers are not liable for delayed or undelivered messages. Do not rely on text messages for urgent or emergency assistance.

7. Privacy

Our Privacy Policy explains how information is collected, used, and shared. Mobile information and SMS opt-in data and consent are not sold or shared with third parties or affiliates for their marketing or promotional purposes. Service providers may process that information to deliver our messages.

8. Changes and contact

We may update these terms by posting a revised version with a new effective date. Changes to the subject matter of text messages require any additional consent that applies; continuing to use the website does not by itself enroll you in SMS. For questions, contact contact@accidentservice.net.
