Privacy Policy

Effective date: September 25, 2026

MDConnect.info ("MDConnect," "we," "us," or "our") respects your privacy. This policy explains how we collect, use, and share information through our website, patient intake portal, and related communications.

Information you provide

When you complete an intake form or contact us, you may provide your name, date of birth, contact information, address, appointment preferences, transportation needs, insurance information, legal representation status, and electronic signature. The intake form also asks about your injury, symptoms, medical history, medications, and prior treatment. We collect the information you choose to submit, including your messaging preferences.

You may browse public pages without completing an intake form. Certain information is required to process an intake or appointment request. Please provide only information relevant to your request.

Technical information and cookies

Our website and hosting or security services may collect technical information such as your IP address, browser and device details, access times, referring pages, and security logs. This information helps operate the website, investigate errors, and prevent abuse. Technical information may identify a device or visitor and is not necessarily anonymous.

We use cookies to maintain authorized portal sessions and support website security. You can manage cookies in your browser, but blocking necessary cookies may prevent access to the intake form or staff pages.

How we use information

  • Process intake submissions and respond to your requests.
  • Coordinate requested appointments, document needs, and related services.
  • Send service-related communications and, when you opt in, non-marketing text messages.
  • Operate, maintain, and secure the portal, and meet applicable recordkeeping or legal obligations.

When information is shared

Intake submissions are sent to designated intake recipients and recorded using email and spreadsheet services. Information may be shared with personnel and providers involved in coordinating the services you request, and with service providers supporting hosting, security, communications, and recordkeeping. Information should be used for those purposes, not for unrelated marketing.

We do not sell personal information or share it with third parties for their own marketing purposes. We may disclose information when required by applicable law, a valid court order, or legal process, or when necessary and legally permitted to address fraud, security incidents, or threats to safety.

SMS privacy and consent

Opting in to text messages is optional. Mobile information and SMS opt-in data and consent will not be sold, rented, or shared with third parties or affiliates for marketing or promotional purposes. We do not transfer your consent to another sender. Service providers that help deliver messages may process this information solely to support our messaging program.

Reply STOP to a message to unsubscribe from that SMS program. Reply HELP for assistance, or email contact@accidentservice.net. See our Terms & Conditions for message frequency, rates, and other messaging details.

Security and retention

We use measures such as HTTPS and access controls to help protect information. No transmission method or storage system can be guaranteed completely secure. Keep portal access codes private, and avoid sending sensitive medical details through ordinary text messages or general support email.

We retain information as needed to process requests, maintain appropriate records, resolve issues, and meet applicable legal obligations. Retention periods depend on the type of information and the purpose for which it is held. Records maintained separately by a medical provider may be governed by that provider's own privacy notices and obligations.

Your choices and privacy requests

You may ask about your information, request corrections or deletion where applicable, or update your communication preferences by emailing contact@accidentservice.net. We may need to verify your identity before acting on a request. Some information may need to be retained for legal or recordkeeping reasons.

Opting out of SMS does not cancel an appointment or an intake request. To change a request, contact us by email.

Other websites

Links to other websites are provided for convenience. Their privacy practices are governed by their own policies. Review those policies before submitting information to another website.

Updates and contact

We may update this policy to reflect changes in our services or practices. The effective date above identifies the current version. For privacy questions, contact contact@accidentservice.net.
